Services include assessment and identification of mental health and psychosocial needs, an individualized plan developed with child and family, and the implementation of that plan. Case Managers works closely to monitor the plan and to make any necessary changes along the way. Our approach adapts to each family’s goals, culture and resources. Case Managers will work with the child and family to develop and achieve measurable goals and outcomes. Decisions, solutions and problem solving are achieved through collaboration, offering children and families thoughtful options for the best outcome.

They provides a comprehensive structure that helps educators build safe, strong, trusting relationships with their students — and these connections pave the way for every student to engage in learning.
